Russia’s ongoing war against Ukraine has triggered a significant refugee crisis in Moldova, placing immense pressure on the country’s already limited resources. Over 10,000 displaced Ukrainians have sought refuge in the Transnistrian (TN) region, where access to modern, qualified long-term care (LTC) services is extremely limited—especially for vulnerable groups such as women, children, the elderly, and people with disabilities. This project aims to support these vulnerable populations by promoting human rights, gender equality, and non-discrimination, while also strengthening good governance through capacity building for local authorities and improving access to inclusive, quality public services.

“Improvement of cross-Nistru cooperation in primary care sectors” project is implemented by the Deutsche Gesellschaft für Internationale Zusammenarbeit (GIZ Moldova) with joint co-financing from the European Union and the German Federal Ministry for Economic Cooperation and Development (BMZ) under the “Supporting Moldovan Communities in Hosting Ukrainian Refugees” project. It aims to enhance confidence between communities on both riverbanks through joint activities in health and social care.

To promote cross-river rapprochement in the health and social care sector, through:
• modernisation of socio-medical sector on the left bank of the Nistru river and progressive alignment with the standards of the right bank through knowledge transfer and cross-river peer-to-peer exchange formats;
• support to establishment of multidisciplinary socio-medical service mechanisms with outreach to both vulnerable layers of population and Ukrainian refugees, within an enlarged geo-graphical coverage through mobile teams and informal sector mobilisation.

• A Multidisciplinary Care Centre (MCC) is established and fully operational in a selected location on the left bank of the Nistru River, while the existing Day Care Centre in Grigoriopol is further strengthened to expand its services.
• The capacity of medical and social care professionals to deliver Multidisciplinary Long-Term Care (MLTC) is significantly enhanced.
• A regulatory framework for palliative care is proposed, drawing on best practices, and efforts are made to progressively align standards across both sides of the Nistru River.
